ACO LEARNING COLLABORATIVE:

Home-Based Care for High-Need, High-Cost Patients

The Institute for Accountable Care and NAACOS supported twelve ACOs through a learning collaborative focusing on developing, implementing and enhancing home visit programs designed to improve care for high-need, high-cost patients. Participating ACOs worked on a diverse range of programs including Hospital at Home; Home-Based Primary Care; Care Transition Home Visits; Community Paramedicine and Community Volunteers. A major issue facing all participants was evaluating the financial implications of their home visit programs. To assist in this process, we developed a return-on-investment (ROI) calculator specifically designed for ACOs based on an extensive review of the literature to identify the range of potential savings achieved by prior programs.
