Learning Collaborative

The Institute for Accountable Care convened an eighteen-month long home-visit learning collaborative in October 2019 to support twelve ACOs as they develop, refine and expand home-visit programs designed to improve care for high-need, high cost individuals. Participants have adopted a range of programs including home-based primary care, hospital-at-home, care transition support, community paramedicine and community health worker initiatives. This work is supported by grants from the Commonwealth Fund, The Scan Foundation, The Robert Wood Johnson Foundation and The John A. Harford Foundation.
