ACO Learning Collaborative:

Addressing Social Drivers of Health

The Institute for Accountable Care and NAACOS are supporting fourteen ACOs through a learning collaborative focusing on developing and implementing strategies to address Social Drivers of Health (SDOH). The collaborative is organized around seven specific content areas that are described below. Presentations and supporting materials for each session will be made available below.

The collaborative is made possible by the generous support of The Commonwealth Fund, The SCAN Foundation, The Robert Wood Johnson Foundation, and the John A. Hartford Foundation.

If you have questions about the learning collaborative please contact Leslie Ziarko Valera at lzv@institute4ac.org.

The initial plan for the learning collaborative curriculum is outlined below. The curriculum is subject to change based on the capabilities and interests of the learning collaborative participants.
