Session 6: CBO Perspectives on Establishing Sustainable Partnerships with Healthcare Providers and Payers

ACOs cannot be successful in addressing social determinants of health without a strong infrastructure of community-based organizations. In this session we provide perspectives from two successful CBOs on how ACOs can partner with them effectively. Both organizations have successfully grown from smaller charitable organizations to larger scale enterprises providing services at scale across a wide geographic area. These CBOs will describe how they have been able to build a sustainable financial mode and discuss challenges and key lessons learned. Both organizations have will discuss their perspectives on the elements of a strong partnership with healthcare providers.
